CONTINUITY CLINIC

Continuity Clinic is the hub of the family medicine residency and is structured to prioritize longitudinal relationships for continuous management of chronic diseases.  It also provides the opportunity for practice-based learning, as residents analyze performance outcomes for their own patient panel and implement systematic changes for quality improvement.

Every resident will have a continuous Continuity Clinic coverage throughout their 3 years of training as follows:

  • PGY-1 residents: will be scheduled one ½ day session per week to begin to build their patient panel.
  • PGY-2 residents: will be scheduled two 1/2 days sessions per week.
  • PGY-3 residents: will be scheduled three 1/2 days sessions per week.

POINT-OF-CARE ULTRASOUND (POCUS)

Each resident will be assigned a POCUS Butterfly iQ+ plus and an iPad mini at the beginning of their residency training.

Residents will receive (CME accredited) evidence-based POCUS educational courses and workshops with ongoing training, evaluation, and mentoring, to ensure you will learn all the necessary skills that are of high importance in primary care settings.

OSTEOPATHIC MANIPULATION MEDICINE (OMM) TRACK

Our Community-based Family Medicine residency program is proud to have Osteopathic Recognition, welcoming both MD and DO residents to join the Osteopathic Manipulative Medicine (OMM) track either in their PGY1 or PGY2 year, with a minimum requirement of 2 years of completion for the OMM track. Residents have the opportunity to take the In-Service Exam (ISE) annually in October, funded by the program, and those who complete at least 2 ISEs can qualify for the Family Medicine Boards, with the option of adding the Performance OMT exam for double board-certification through the American Osteopathic Board of Family Physicians (AOBFP).

During residency, OMM track residents utilize one elective rotation each year as their core OMT rotation, integrating osteopathic skills into real-world scenarios and utilizing the Fascial Distortion Model (FDM) in treatment. They also engage in learning osteopathic history and theory, participate in the OMT Clinical Journal Club, and provide patient care at the UIWSOM OMT clinic and Communicare West during OMT Continuity Clinic monthly.

RubiconMD is a very rich E-Consult platform that is built into our EMR system, which allows you to empower clinical decision-making and diagnoses with a convenient, and same-day access to board-certified specialists, to allow you to deliver the best primary care experience to all patients and also earn CME credit at the same.

This amazing service is FREE for all, including both insured and uninsured patients.

IN-HOUSE PHARMACY

Our CommuniCare pharmacy team provides a personal level of management and counseling needed for safe, effective, and affordable treatment.

In addition to counseling, our pharmacy provides the following services to CommuniCare patients:

Sliding fee discounts, based on income, as an option for patients who qualify, as well as a $0 co-pay for brand prescriptions for qualifying individuals, regardless of insurance status.

FREE delivery of all medications directly to your patient’s home or location of choice in San Antonio. They can also arrange for medications to be delivered to our Continuity Clinic for patients to pick-up.

POST-GRADUATION JOB OPPORTUNITIES

One of the primary objectives of our residency program is to ensure successful job placement for our residents prior to graduation.

During the PGY-2 year, a thorough selection process led by our Program Director, Associate Program Director, and esteemed Faculty Members identifies the most outstanding resident(s) deemed to be an ideal match for employment at CommuniCare.

Residents who are recruited by CommuniCare are offered an exceptional contract featuring comprehensive benefits and monthly stipends upon signing. Additionally, employment with CommuniCare qualifies individuals for participation in the Physician Education Loan Repayment Program (PELRP).
